To request a counseling session:

• Complete page 1 only of the Counseling Information Form. Make sure it is signed and dated (blocks 13 and 14).
• Call the Small Business Development Center at (334) 844-4220 to schedule an appointment with one of our counselors.

• Either mail or fax the form to the Small Business Development Center. One of our counselors will contact you to schedule your appointment.
